Commonwealth Home Support Program Services

Auscare’s Commonwealth Home Support Program (CHSP) is available for older Australians who require entry-level support services to live independently and safely at home.

Funded by the Australian Government, CHSP services work with you to maintain your independence rather than doing things for you, supporting a more independent and connected life.

What the CHSP Offer?

Home Modifications

Auscare’s home modifications services will require a referral from Commonwealth Home Support Programme. This program can improve your independence and confidence with your activities of daily living, and increase your safety and security in your home. 

Allied Health

Auscare’s experienced allied health teams of, physiotherapists, occupational therapists and occupational therapy assistants, can assist you to improve your mobility and wellbeing, including recommendations for assistive technology.

Cottage Respite & Day Centres

Auscare Cottage Respite service is funded by the Community Home Support Program offering a homelike, friendly and cheerful atmosphere with flexible respite options for family carers looking after a loved one. The wellbeing of carers is important and by offering short breaks can assist in your caring role. While your loved one can enjoy activities in a safe and caring environment, while you take some time out. Depending on your needs our options may include day, evening, overnight or 24-hour care.

Social Supports & Excursions

Social Support Group operates in various locations in Metropolitan Perth, providing social group outings and/or centre based activities to our older Australians. These activities are designed to develop, maintain or support social connections.

For those who would like to go on a day adventure, we provide the transport. All you need is to bring lunch  or you can choose a meal to enjoy. We can arrange a local restaurant or cafe, catch a movie or even try something new with the safety and support of our team

Are you a carer? It is important for you to take a break from your caring role every now and again. Auscare’s Social Support Group / Centre Based Care aims to give you both the time and space to do the things you enjoy most. Ask us how!

Am I eligible?

If you’re over 65, and/or Multicultural, Aboriginal or Torres Strait Islander over 50, you could be eligible for government-subsidised support.

What does CHSP cost?

If approved for CHSP, much of the cost will be covered by your funding.

You’ll only be asked to contribute a fee for the services you receive.

If you are experiencing financial hardship, you might be able to have this waived.

Access Auscare’s CHSP Programme

CHSP Programme connects and keeps you safe in your home or community.

To access Auscare’s CHSP easily, we’ve outlined the four steps you and/or your assisting family members, carers or loved ones should follow:

Contact My Aged Care online or on 1800 200 422 to discuss your need for services. This discussion will trigger a Regional Assessment Service (RAS) review.

After your RAS assessment, you’ll receive a letter that indicates what CHSP service is recommended,

You’ll receive a confirmation letter when your CHSP has been assigned. It contains a referral code that allows you to access services.

Call Auscare Community to assist in activating your CHSP service. Our friendly team will ensure you receive expert advice and the best level of support
